OPINION QUESTIONS
What is the most frequent cause of dry eye amongst patients you have treated?
Most common is Post –menopausal women. Most misdiagnosed (We have a national secondary referral practice): Blepharitis, MGD, DTS
What advice do you most frequently find yourself giving to dry eye patients?
Stop all the eye drop cocktails that you have been asked to use. Always ask your treating physician why they are starting a particular therapy and what is the diagnosis.
Do you have a dry eye "hot button" - for example, is there a condition you feel is often overlooked or misdiagnosed, or a treatment you feel is sometimes used inappropriately?
I classify these into:
Lid based (Secondary dry eyes, Blepharitis, MGD);
Cornea based: Corneal dystrophies and irregular surfaces including Form Fruste Keratoconus; and
Conjunctiva based: Conjunctivochalasis.
This is further classified but that is all part of my workup and educating patients so accurate diagnosis leads to accurate therapy (medical or surgical).
What makes you exceptionally well qualified to help dry eye patients?
The ability to understand the entire ocular surface as a part of ocular millieu as well as a visual determinant. Just like in Laser Vision surgery. One cannot just look at one aspect of ocular health/ technique or technology and disregard the connection between others. Therefore this entire speciality I have re-defined as CORNEOPLASTIQUE™.
What are some of your favorite products for dry eye (Rx, OTC, consumer)?
I do not have favorites. I have appropriate treatments for accurate diagnoses. Dry eye is a SCIENCE and its treatment is an ART. This is what I keep teaching eye surgeons and optometrists worldwide.
